In-Network

We are happy to assist you as best as we can in determining your benefits and eligibility, but it is ultimately your responsibility to confirm your coverage, deductible, and copay before attending your first appointment. If your insurance company declines coverage you will be responsible for the full cost of all services rendered.​

Stefan Chase accepts:

  • Select Health
    (Most plans with some exceptions)    ​

Bryce Barker accepts: 

  • Select Health

  • BCBS of Utah (Blue Cross Blue Shield)

  • EMI (Educators Mutual Insurance)

  • PEHP (Public Employees Health Plan)

  • DMBA (Deseret Mutual Benefit Administrators)

Out-of-Network

If we do not accept your insurance, you may have Out-of-Network benefits that can reduce the cost of therapy. Mentaya can help you do this! Use the tool below to see if you qualify for reimbursement for therapy from your insurance company.

Note: Mentaya's goal is to save you time and money. It's completely optional and our therapists do not benefit in any way from your participation.

Mentaya is perfect if you:

  • Have Out-of-Network benefits.

  • Feel overwhelmed by superbills and insurance.

  • Have submitted superbills but failed to get any reimbursement.

  • Simply want to skip the hassle of paperwork!

 

Here's how it works:

  1. Sign up for Mentaya here.

  2. Our practice will enter your sessions into the platform.

  3. Mentaya submits the claim and handles any insurance follow-up.

  4. You get reimbursed by insurance!

 

Mentaya charges a 5% fee per claim, which includes handling any paperwork required, dealing with denials, and calling insurance companies. It's risk-free: They guarantee claims are successfully submitted, or a full refund of their fees.

The other option you have is by doing it yourself with a receipt we can provide called a "superbill." It's your responsibility to find out how to submit the superbill to your insurance and handle any logistics associated with the process.
